Once scheduled, you will have access to an online patient portal. Through the portal, you will complete brief questionnaire(s) and review documents summarizing clinic policies before your appointment. If the paperwork is not completed 24 hours before your appointment, you may be rescheduled.

• Clinical interview (up to an hour, telehealth or in-person): You will be asked about current cognitive and psychological symptoms, medical and mental health history, educational attainment, and other background.
• In-person testing (typically 1-4 hours): You will be asked to complete a series of tests, which will be pencil-and-paper tests and/or computer tests. This may include cognitive tests (e.g., memory, attention, processing speed), academic tests (e.g., math, spelling, reading), and/or psychological questionnaires (e.g., mood, personality) depending on the goals and needs for the evaluation.

• This will be scheduled after you finish the assessment. During the appointment, you will review the results and recommendations with the provider. Following the appointment, a copy of the report will be provided to you and your referring provider (typically within 2 weeks of your feedback appointment unless communicated otherwise).
