If the referral is accepted, you will be contacted for scheduling (or placed on a waitlist). Once scheduled, you will have access to an online patient portal and will be asked to complete brief questionnaire(s) and review documents summarizing clinic policies before your appointment. If the paperwork is not completed 24 hours before your appointment, you may be rescheduled.

• Cherry Hills Neuropsychology LLC is contracted with Aetna, Medicare, Moda, PacificSource, Providence, and Regence BCBS. If not listed, please contact us to determine if we are in-network with your insurance.
• If Cherry Hills Neuropsychology LLC is in-network with your insurance, we will obtain prior authorization if required and file a claim through your insurance. You are responsible for verifying benefits and coverage with your insurance company, and for paying any copays, co-insurance and/or deductibles as required by your insurance policy.

• You are responsible for paying for services in full. You will be provided with a Good Faith Estimate prior to your appointment, which will summarize billing codes, diagnostic codes, and range of fees. You will be responsible for confirming coverage and submitting documents to your insurance company if you choose to seek reimbursement for out-of-network services. Following completion of your evaluation, you will be provided with a “Superbill” detailing billing codes, diagnostic codes, and payment, which you can submit to your insurance for reimbursement if you choose.
